Undergraduate Advising Form

Please print the form linked here before coming in for advising.  Fill in the form as follows:

  1. For each course you took at another college or university and for which you received official transfer credit, write “T” next to its title.
  2. For each course you have taken at UMass Lowell, write the grade you received in that course next to its title.
  3. For each course you are taking this semester, write the grade you expect to receive next to its title and circle that grade.
  4. For each course you are planning to take next semester, put an asterisk (*) next to its title.
    • If the course is an elective and its exact title does not appear in this table, please write in its title.

This will make your advising go much more quickly and smoothly, and you’ll get far better advice!  Thank you.

You may also want to use the Excel CS CUM Calculator to check your CS CUM, which must be at least 2.5 to graduate.
